Improving quality of life and preventing hospital readmissions

The cardiac care program with BJC Home Care is for patients recovering from cardiac-related surgeries or diagnostic procedures, or who are living with chronic cardiac disease. Patients receive highly specialized cardiac monitoring, education and personal care in their homes. The program helps patients understand their condition or disease, and its management.

The cycle of home to hospital to home again can be frustrating for the patient and his or her family. Care through BJC Home Care’s cardiac program can break that cycle. Patients learn how to reduce their risks and manage their medication, diet and activity levels. They also learn to identify symptoms early, reducing their chances of having to be hospitalized again.

Care is directed by the patient's physician and provided by experienced cardiovascular nurses, who do a complete assessment of the patient within 48 hours after receiving a referral. The nurse will work with the patient, physician and family to develop a home treatment plan. A specialized team, including a dietitian, physical and occupational therapist, provide care to promote a proper recovery and a return to daily activities.
