Cardiac Care
The cardiac care program is for patients recovering from cardiac-related medical interventions or diagnostic procedures, or who are living with chronic cardiac disease. Patients receive highly specialized cardiac monitoring, education and personal care in their homes. The program helps patients understand the cardiac condition or disease, and its management.
Care is directed by the patient's physician and provided by experienced cardiovascular nurses, who initiate a complete post-discharge patient assessment within 48 hours of referral, or sooner if ordered by the physician. The nurse collaborates with the patient, physician and family to develop an individualized home treatment plan that incorporates care guidelines, protocols and wellness goals.
