To refer a patient to BJC Home Care Services:

• Call the Central Referral Center at 314.747.8600 or toll-free 888.BJC.HOME
• Print our referral form, complete it and fax to 314.747.6093; you will receive a call confirming the receipt of the fax
• Use our online referral form

BJC Home Care Services will notify you after a home visit has been made to your patient.
